Freedom of Information and the Protection of Privacy at Casey House

Beginning on January 1, 2012, Casey House Hospice is subject to the Freedom of Information and Protection of Privacy Act (FIPPA). Casey House Hospice has consistently promoted a culture of openness and accountability within the organization. To ensure full compliance with the Act, the Casey House Hospice has appointed a Freedom of Information Officer that will process freedom of information requests and will work with the Privacy Officer to oversee the protection of privacy.

About FIPPA
The purpose of FIPPA is two-fold:
1) To make public bodies more transparent and accountable by providing a right of access to information to government records (freedom of information); and
2) to protect the privacy of individuals with respect to personal information about themselves in the custody or the control of public bodies (protection of privacy).

Requesting Access to Casey House Hospice Records
There are two categories of FOI Requests:

Request for one’s own information: Only an individual (or his or her personal representative) can make a personal information request for access to his or her own personal information.
General records request: This includes all other requests, including requests for the personal information of other individuals. Any person or organization can request access to general records.

You do not always have to file a FOI request to obtain records. Casey House proactively discloses information on our website and we encourage you to visit www.caseyhouse.com to see if the information you are looking for is available.

If the information is not immediately available, please feel free to contact staff at Casey House to see if the information is readily available. It may be possible to receive the information without having to make a formal request.

If the information is not readily available, you can request information by filling out the attached form (PDF) or write a letter that provides sufficient detail to identify the record or records you are requesting.

Any request for information that involves all the following elements will be treated as an FOI Request:

It is a request for access to information;
It is made in writing (a simple letter will suffice);
It is submitted to Casey House Hospice;
It provides sufficient detail to enable an experienced employee of Casey House Hospice to identify the Record(s) requested; and
It is accompanied by the $5 application fee (i.e., a cheque payable to Casey House Hospice).

FIPPA requires that all formal requests be accompanied by a $5.00 application fee. There may be other fees applicable depending on the volume or complexity of the request. Casey House will contact you to inform you if the fees are in excess of $25.00. You have several options to proceed at that time: 1. You may wish to further define the request to pinpoint the exact record(s) that you want; 2. You may request to view the records in a viewing room – there is no additional charge for this viewing; or 3. You can request that Casey House waive the fees.
