Care Co-ordination
Consistent care co-ordination is directed by one dedicated professional who helps an individual reach their personal and health-related goals in the care delivery model.
Often, the most effective care co-ordinator will facilitate everything, as appropriate, as a client navigates the health care and social service systems. They connect clients to the services and programs at every stage of their disease. They monitor the availability and quality of resources and bring attention to gaps.
The care co-ordinator may simply provide information to their client, actively link them to a service provider, make appointments on their behalf, or arrange for a volunteer to transport them. The degree of involvement is determined by the clients' situation and the needs they identify.
