A Model of Care Built on Core Values

Core values inform our Model of Care and everything we do. The goal is to help people living with HIV/AIDS develop the knowledge, skills & experience they need to direct their own lives.

Constant evaluation ensures that we deliver:

Needs-Based Care
focusing on what an individual needs at a specific time in their illness;

Care Co-ordination
using a systematic approach to support an individual in achieving their medical and personal goals;

Inter-Professional Collaboration
working together across disciplines to ensure the most integrated and comprehensive care plan is developed and delivered; and,

Comprehensive Programs
building capacity through partnerships with service and program delivery experts so that individuals can access what they need, when they need it.

Of course none of this would be possible without a deep commitment to teamwork among the excellent people who make it happen.
