Donation of Securities or Shares

Instructions:

If you would like to make a gift of securities to Casey House Foundation, please follow these steps:

  1. Download a copy of the Letter of Authorization Form. We also suggest that you advise your broker or financial advisor of your intent to transfer the securities/shares to Casey House Foundation.

  2. Complete the form in its entirety.  Fax or mail the form to Casey House Foundation at (416) 962-5595, attention Kristin Thomas, and to your broker. Casey House Foundation’s investment advisor requires the instructions from the Foundation before accepting the shares into Casey House Foundation’s Account.

  3. Instruct your broker to initiate the transfer of your shares into Casey House Foundation’s account # 439-3123-1-2.
Please Note:  A donation of Mutual Funds involves a multi-step process which may take anywhere between several days to several weeks to process depending on the mutual fund company and the type of security being transferred.
